   Evaluation Devices Guidelines

PURPOSE: To give personnel access to a variety of devices which may be used for evaluation of students, diagnostic trials, or inservice. Evaluation may occur in all environments, including but not limited to school, home, or other facility involved with the student. The SERRC will determine and monitor the appropriate use of the devices.

1. Devices will be loaned to or through the therapist designated responsible for the devices.

2. The designated person is responsible for each device borrowed.

3. Devices will be scheduled on a first come-first served basis.

4. Each device may be borrowed for a three week period. The loan may be extended twice for an additional three weeks each time if the device is available. Extensions cannot be prearranged. If the device is requested during the renewal period by another individual, the device must be returned immediately. Contact Terri Densford to arrange an extension.

5. Each device must be picked up and returned at the SWO SERRC, 1301 Bonnell, 3rd Floor, Cincinnati, Ohio 45215 Phone - 513-563-0045, Ext 10

6. The borrower should complete and return all parts of the Assistive Device and Evaluation form.

7. Missing or damaged items should be reported by borrowers. SWO SERRC will maintain necessary service contracts for the equipment and/or provide routine equipment repairs.

8. Repairs necessary as a result of negligence, inappropriate use, or mishandling will be the responsibility of the borrower.

9. Devices needing repairs should be returned to SWO SERRC as soon as possible. SWO SERRC will reschedule the loan of the device during the next available time period.
