IDEA 2004 defines Transition services as a set of coordinated activities that are designed within a results-oriented process that focuses on academic and functional achievement, is based upon the individual student’s needs and includes instruction, related services, community experiences, and the development of employment and other post-school adult living skills.
Federal regulations require the inclusion of a transition plan in the IEP when the student turns 16. However, Ohio continues to support the idea of starting transition at age 14. It is never too early to start the transition process.
